Bio

Pam Lincoln-Giang, PharmD, BCGP, currently serves as the senior director of pharmacy for WelbeHealth, where she is responsible for overall pharmacy strategy and development of the pharmacist role within the PACE model of care. Her prior experience spans a multitude of managed care venues, including risk-based provider groups, home-based provider groups, PBMs, and health plans. Her areas of interest include value-based care, quality, HEDIS and CMS Star measures.
Pam received her BS in developmental and cell biology from the University of California, Irvine, and completed her PharmD degree and ASHP-accredited PGY1 residency with the University of Southern California. She is also a board-certified geriatric pharmacist.

Bio

Hindu Rao, PharmD, APh, BCACP, is a clinical assistant professor of pharmacy practice at the Chapman University School of Pharmacy (CUSP) and an ambulatory care pharmacist at Providence Medical Foundation (PMF). She specializes in chronic disease management, specifically diabetes, hypertension, and dyslipidemia management. Hindu received her PharmD degree at the University of California San Francisco, where she became a member of the Rho Chi Pharmacy Honor Society. She completed her postgraduate pharmacy residency training at Cedars Sinai Medical Network and CUSP, with a focus in ambulatory care practice. During her residency, she also obtained a teaching certificate through CUSP.
Hindu is passionate about educating the next generation of pharmacy leaders. Currently, she teaches first-year student pharmacists at CUSP and is a preceptor for third-year students in her ambulatory care clinic at PMF. Her other interests include developing and advocating for pharmacist-led services within medical homes and accountable care organizations. She also aims to expand the use of health technology in pharmacist-led patient care services.

Sarah Aldrich Renner, PharmD, BCACP, is a clinical assistant professor (associate lecturer) at the University of Toledo College of Pharmacy and Pharmaceutical Sciences, and practices as a clinical ambulatory care pharmacist within the University of Toledo General Internal Medicine Clinics. She earned her PharmD degree at the Ohio Northern University Raabe College of Pharmacy before completing 2 years of residency at Michigan Medicine. Her PGY1 residency was inpatient-focused, with her PGY2 residency specializing in ambulatory care practice. Currently, Sarah utilizes collaborative practice agreements to manage chronic diseases including diabetes, hypertension, dyslipidemia and smoking cessation. She serves as a medication resource and collaborative team member within her outpatient practice.
As a faculty member, Sarah engages in experiential and didactic teaching with student pharmacists, pharmacy residents, and medical residents. Her research and practice interests include quality improvement, population health, and transitions of care. She works actively to engage in patient and pharmacy advocacy through her membership with APhA and the Ohio Pharmacist Association.

Saba Syed, PharmD, MS, EMBA, BCACP, BCGP, is vice president of pharmacy at Aledade—the largest network of independent primary care in the country, serving more than 2 million patients—where she is responsible for all aspects of pharmacy strategy and performance. Saba is a skilled clinician and lifelong learner with a track record of execution at the intersection of health care services, value-based care business operations, technology, and data analytics. She has successfully developed, launched, and managed multiple clinical service lines with remote, diverse teams at fast-growing health care technology and value-based care start-ups. Additionally, she has trained the next generation of pharmacy leaders as the residency program director of a PGY2 population health and data analytics pharmacy residency.
Saba has more than 16 years of experience in community pharmacy, mail-order pharmacy, managed care, primary care, population health, value-based care operations, project management, product development, and leadership. This includes experience with prior authorizations (Medicare and Medicaid), Medicare formularies, medication reconciliation, transitions of care, medication therapy management, care management, accountable care organizations (ACO), Medicare quality measures, and medication adherence. Her experience across the ambulatory care pharmacy and value-based care continuum gives her a unique insight and perspective into the challenges and opportunities facing pharmacists looking to make an impact within ACOs.
