Transitions of Care SIG Community

Mission

The mission of the APhA-APPM Transitions of Care Special Interest Group (SIG) is to create a professional network of pharmacists and technicians who are working to overcome the transitional challenges and enhance the collaboration among pharmacists in all practice settings as they provide care to patients during transitions to/from various health care settings.

Purpose

The purpose of the APhA-APPM Transitions of Care SIG is to provide a community where pharmacists and technicians in all practice settings can navigate the complex health care system by sharing ideas on how to collaborate in optimizing transitions of care.

  • Communications Committee

    • The purpose of the Committee is to utilize the SIG ENGAGE platform to lead professional discussion and facilitate collaboration amongst the TOC SIG community.

  • Education Committee

    Chair(s): Savanna San Filippo & Marissa Cavaretta  

    • The purpose of the Committee is to coordinate CPE and/or non-CPE webinars over the academy year.

  • Practice Committee

    Chair(s): Reba Forbess  

    • The purpose of the Committee is to review and update 1-2 TOC resources on the APhA TOC page, and to identify the need for tools/resources for pharmacists and pharmacy extenders in various transitions of care practice settings.
